Duty To Warn and Confidentiality
Confidentiality is of utmost importance in a therapist and client relationship. The sacred relationship formed between the therapist and client forms the bedrock of psychotherapy; the trust that all the client’s and therapist’s secrets will be kept within our 4 walls. Without this trust, no real work can be done.
However, there are cases when we have to breach this sacred relationship to follow what the law mandates us to do.
The few exceptions are:
- Impending harm to the client, like suicide
- Impending harm to other people, like the client having intention to harm another person
Why?
In 1976 in America, there was a man named Prosenjit Poddar who met a girl, Tatiana Tarasoff, in university and had a brief romantic relationship with her. The man interpreted this relationship as serious but the same view was not shared by the girl.
He felt rejected and went into deep depression. At that point in time, he was seeing a therapist and disclosed his intention to harm her during the course of therapy.
Eventually, he managed to brutally murder the girl after a period of detailed planning. During the time prior to the murder, he had stopped seeing the therapist and was probably well enough to escape detection by people around him.
The judge deemed that the health professionals did not carry out their due diligence in warning the potential victim of the death threats.
Ever since
Ever since then, mental health professionals have become more vigilant in trying to pick up risk factors for behaviour that may cause harm to the patient and/or to others.

Dr Ang Teng Soon Paul
Dr Paul Ang is a family physician or what is commonly known as a general practitioner (GP) in Punggol, Singapore. He founded Zenith Medical Clinic at Punggol West in 2014. He is keen to improve the way primary care (or GP services) are delivered. He wants to bring great, quality and affordable healthcare to the residents in Punggol, Singapore.
His professional qualifications are Bachelor of Medicine and Bachelor of Surgery (MBBS) Singapore, Graduate Diploma in Family Medicine (GDFM), Graduate Diploma in Mental Health (GDMH), Membership of the Royal College of Surgeons (MRCSEd) and accredited family physician. He is currently pursuing the Masters in Liberal Arts (Digital Media and Design) at Harvard Extension School.
His clinical interests include Ear, Nose, Throat (ENT), Mental Health and wellness and Paediatrics. Previous appointments include deputy director of class primary care network and director in MaNaDr mobile health.
Other significant past achievements include President Scout Award in 2004 awarded by the late President SR Nathan.
He strongly believes that a sustainable enterprise must always contribute back to the society in both tangible and intangible ways. As a medical student, he founded the Project Sa’Bai with a close friend and with a lot of help from two experienced family physicians (Dr John Lee and Priscilla Lee).
He believes in a healthy lifestyle. He can be spotted cycling around the Punggol and Sengkang areas with his two young boys and spouse. (His favourite by the way are Coney Island, Punggol Waterway Park and Sengkang Riverside Park. Thank you NParks!)
Dr Ang Kai Kok
Dr Ang Kai Kok is a caring and approachable general practitioner (GP). He is a partner in Zenith Medical Clinic (Punggol West). He advocates health education and strives to guide the patients in Punggol in making personalized care plans based on strong scientific evidence.
His professional qualification is Bachelor of Medicine and Bachelor of Surgery (MBBS) Singapore and the Graduate Diploma In Family Medicine (GDFM).
Being a keen educator, he was commended as a “Role Model” to the medical student class of 2014 by National University of Singapore. He was also the founder of Raffles Junior College – British Power mentoring program back when he was a student. He was also an active participant of the above mentioned Project Sa’Bai in his medical student years.
He strongly believes that healthcare must be holistic and GPs must constantly upgrade themselves and remain at the center of the current complex medical system.
